Technique B: The Crestal Sinus Lift (For Mild to Moderate Bone Loss)

If you already possess a moderate amount of native bone (e.g., 5 to 7 millimeters) but just need a little extra height to accommodate the implant, the much faster “Crestal” approach is used. This is often called a One-Stage Procedure.

  1. Drilling the Implant Site: The surgeon begins by drilling the vertical channel into the gum exactly where the future implant will go. They stop drilling just a fraction of a millimeter shy of the sinus floor.
  2. Tapping the Floor: Special instruments are used to gently “tap” and fracture the paper-thin floor of the sinus, pushing the membrane upward through that single vertical hole.
  3. Simultaneous Placement: A small amount of bone graft is pushed up into the space, and the actual titanium dental implant is screwed into place immediately. The implant itself holds the graft and the membrane up while the bone heals around it over the next 4 months.

Crucial Behavioral Restrictions (The “Sinus Precautions”): For 3 to 4 weeks after the surgery, you must absolutely avoid any actions that create high pressure within your nasal or sinus cavities:

  • Do not blow your nose. Gently dab it with a tissue if it runs.
  • If you must sneeze, do so with your mouth wide open to expel the pressure forward, rather than forcefully through your nose.
  • Avoid heavy lifting or strenuous exercise that causes you to grit your teeth and strain.

Most patients are surprised by how manageable the recovery is. The procedure itself is completely painless. Post-operatively, you will experience some swelling and possible bruising for a few days, but the pain is generally mild and easily controlled with standard pain relievers.

What happens if I sneeze after a sinus lift?

Sneezing violently or blowing your nose can create massive pressure within the sinus, potentially dislodging the newly placed bone graft or tearing the healing membrane. You must avoid blowing your nose for several weeks, and if you must sneeze, do so with your mouth wide open to release the pressure.

Can the implants be placed at the exact same time as the sinus lift?

It depends on your starting anatomy. If you have at least a few millimeters of your own bone remaining to hold the implant stable during surgery, we can do both simultaneously (a One-Stage procedure). If the bone is paper-thin, the graft must heal and solidify for 4-6 months before implants can be placed (a Two-Stage procedure).

Will a sinus lift change the way my voice sounds or my ability to breathe?

No. While the maxillary sinus cavities are large air-filled spaces, a sinus lift only elevates the absolute bottom floor of the cavity by a few millimeters. This does not noticeably reduce the volume of the sinus or alter its function, meaning it will not affect your voice, breathing, or sense of smell.

What if the sinus membrane accidentally tears during the lift surgery?
